Before applying for the course you must discuss time and workload implications with your manager.  If you are both in agreement that you should attend then ask your manager to complete the manager’s form. Manager's Form in doc format

Send your application form along with your managers form and full payment to:

If your form is received illegible, if your application is incomplete, a place can not be reserved and your application will be returned for amendment. This delay may result in you loosing your place.
